Wellness Program

 

The Wellness Program...

  • Encourages healthy behaviors
  • Is a points-based system
  • Allows employees and their spouses/Financially Interdependent Partners (FIPs) to earn a rebate on their medical premiums
  • There are no biweekly premiums for the Wellness Program itself
  • Employees who participate in the Wellness Program must be enrolled in a UA Choice Medical Plan and pay biweekly premiums on their medical plan

Wellness Program Details

8 Points Total

  • Total of eight (8) points required
    • Two (2) required activities
    • Six (6) voluntary activities
  • Spouse/FIPs must earn their own points
  • Points must be earned on or before June 30 each year
  • Review the Program Guide for more information

2 Priority (Required) Activities

  • Health Check Survey (online)
    • Click on "Health" (the heart icon) in the ribbon at the top and click "Take Survey" under "Health Check"
  • Biometric Screening (in-clinic or mail-order kit)
    • Click on "Benefits" (the hand icon) in the ribbon at the top > LabCorp > Start Now > SSO to LabCorp landing page > select screening option

Collect an Additional 6 Points


Complete Registration | Complete all steps for a maximum of 1 point

  • First login to mobile app
  • Add a profile picture
  • Add 5 friends

Worksite Health Challenges | Maximum of 2 points

  • Join the company challenge - 1 point
  • Win the promoted healthy habit challenge - 1 point

Telephonic Coaching | 2 points

  • Complete four (4) coaching sessions - 2 points

Preventive Care (based on age/sex) | Maximum of 5 points

  • Multiple preventive care points available depending on age/sex
  • Each preventive care screening - 1 point to a maximum of 5 points

Ideal/Improved Measurements | Maximum of 1 point

  • Meet 3 out of 5 biometric measurements for the current years biometric screening - 1 Point
  • Reasonable Alternative: Employees can complete a Journey as a reasonable alternative to meeting the 3 out of 5 biometric measurements - 1 point

Livongo powered by Teladoc | Maximum of 3 points

  • Hypertension - 1 point when complete all 3
    • Enroll
    • Activate device
    • Set blood pressure baseline with device
  • Diabetes - 1 point when complete all 3
    • Enroll
    • Activate device
    • Check blood glucose with device

  • Weight Management - 1 point when complete all 3
    • Enroll
    • Activate device
    • Check weight with device

20-Day Triple Tracker | 1 point each time, Maximum of 4 points

  • 10,000 steps, 30 active minutes, and 30 workout minutes in 20 days
  • One point each 20 day stretch with a maximum of 4 points

Complete a Journey | 1 point each Journey, Maximum of 2 points

Earned but Locked Points

Once employees are logged into the platform, there is a 鈥淩ewards鈥 drop down in the upper right. This will show the number of days remaining in the plan year along with the total points earned so far. 

Because the Health Risk Assessment (called 鈥淗ealth Check鈥 with Personify Health) and the Biometric Screening are required activities, these must be completed before any 鈥渓ocked鈥 points are officially rewarded. Employees will be able to see 鈥渓ocked鈥 points, but they will not be officially added to the point total until the required activities are completed. 

Must be Enrolled on UA Choice Plan

  • Must be enrolled in respective UA Choice medical, dental and/or vision plan to earn points for those visits
  • No points awarded for visits against insurance coverage on a non-UA Choice plan

Rebate - Employee Requirements

To be eligible for the rebate, employees must be able to answer "yes" to all of the following questions each November when the rebate is rewarded. 

  1. I have completed the wellness program - including all required activities and additional points.
  2. I am a current, benefit-eligible employee during the pay period that the rebate is being awarded.
  3. I am enrolled in a medical plan (Premium, Basic, or HDHP) during the pay period that the rebate is being awarded.鈥

Rebate - Spouse/Financially Interdependent Partner (FIP) Requirements

To be eligible for the rebate, spouses/FIPs must be able to answer "yes" to all of the following questions each November when the rebate is rewarded. 

  1. I have completed the wellness program - including all required activities and additional points.
  2. My spouse/FIP is a current, benefit-eligible employee during the pay period that the rebate is being awarded.
  3. I am enrolled in a medical plan (Premium, Basic, or HDHP) with my spouse/FIP during the pay period that the rebate is being awarded.鈥

Rebate - On Dental/Vision Only? 

Employees who are not enrolled in a medical plan (Premium, Basic, or HDHP) are not eligible to participate in the program or receive a rebate.

Earn 12 Points

Earn 12+ points and be entered to win a gift card of up to $500 at select retailers.

 

Enrollment & Arrears Information

  • 鈥婤enefit-eligible employees working a minimum of 20 hours per week and are enrolled in a UA Choice Medical Plan
  • Spouses/Financially Interdependent Partners (FIPs) of employees as long as they are enrolled in a UA Choice Medical Plan
  • Temporary Employees are not eligible (visit the temporary employee webpage for eligible benefits)
  • No enrollment required (as long as enrolled in a UA Choice Medical Plan)
  • Login to Personify Health to get started

When logging in for the first time, be sure to use the employee's UA ID with a trailing "SP" (i.e. 30012345SP).

First Day of Coverage in the Wellness Program

  • May be a delay in reporting between UA > Premera > Personify Health
  • Allow for 4 weeks after employee start date before logging into the Wellness Plan for the first time
End of the Pay Period
  • Coverage will end at the end of the pay period in which an employee (1) separates from the University or (2) ends coverage due to a life event
This is a free/voluntary benefit provided to University employees. There are no arrears.
